Job Summary

We are seeking a compassionate and dedicated Registered Nurse - Home Health to provide high-quality nursing care to patients in their homes. The ideal candidate will possess a strong clinical background, excellent communication skills, and a commitment to enhancing the quality of life for our patients. This role involves working independently while collaborating with a multidisciplinary team to deliver comprehensive care.

Duties

• Conduct thorough assessments of patients’ health status, including vital signs and medical history.
• Develop and implement individualized care plans based on diagnostic evaluations and patient needs.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
• Provide catheter care and assist with cardiac catheterization procedures when necessary.
• Perform triage for patients requiring urgent care and coordinate referrals to appropriate services.
• Offer education and support to patients and their families regarding health management and disease prevention.
• Document all patient interactions accurately in electronic health records (EHR).
• Collaborate with healthcare professionals in various settings, including clinics, ERs, and ORs, to ensure continuity of care.
• Participate in travel nursing assignments as needed to support patient care across different locations.

Qualifications

• Current Registered Nurse (RN) license in the state of practice.
• Experience in senior care or home health preferred.
• Proficiency in performing suture techniques and other clinical skills relevant to home health nursing.
• Familiarity with diagnostic evaluation processes and the ability to interpret results effectively.
• Strong interpersonal skills with the ability to build rapport with patients and their families.
• Ability to work independently while managing time effectively in a home health setting.
• Willingness to travel as required for patient assignments.

Pay: $45.00 - $50.00 per hour
