Elite Professionals Home Care is hiring a Registered Nurse (RN) for TWO GREAT OPPORTUNITIES:

FULL-TIME RN CASE MANAGER

• $80,000 annual salary + performance bonus
• Monday–Friday, 8 AM–5 PM
• Rotating on-call approximately 1 weekend/month
• Clinical leadership, education, and growth opportunities

PART-TIME WEEKEND RN

• $40/hour + mileage
• Every other weekend
• Home health experience required
• Weekday training required

WHAT YOU'LL DO

• Manage patient care from admission through discharge.
• Complete OASIS assessments, admissions, recertifications, resumptions, and discharges.
• Develop and oversee individualized Plans of Care.
• Perform home visits and monitor patient progress.
• Collaborate with physicians and interdisciplinary team members.
• Provide wound care and wound vac management when needed.
