FULL-TIME RN CASE MANAGER | $80,000 + BONUS

• $80,000 annual base salary PLUS performance bonus
• Monday–Friday, 8:00 AM–5:00 PM
• Rotating on-call approximately one weekend per month
• Structured orientation, clinical support, and professional growth opportunities

PART-TIME WEEKEND RN | $40/HOUR + MILEAGE

• $40 per hour PLUS mileage
• Every other weekend
• Home health experience required
• Some weekday training required

As an RN Case Manager, you'll play an important role in coordinating patient care, supporting clinical excellence, and helping patients achieve the best possible outcomes—all while working with a supportive, collaborative team.

WHAT YOU'LL DO

• Lead Patient Care: Manage care from admission through discharge, ensuring patients receive quality, individualized services.
• Put Your Clinical Expertise to Work: Complete comprehensive assessments, OASIS admissions, recertifications, resumptions of care, and discharge assessments.
• Develop Plans of Care: Create, implement, and oversee individualized care plans that meet patient needs.
• Collaborate Across the Care Team: Coordinate with physicians, therapists, social workers, home health aides, patients, and families.
• Support and Educate Nurses: Share your knowledge, provide clinical guidance, and help team members strengthen their skills.
• Make a Difference in the Home: Perform home visits, monitor patient progress, and help patients remain safe and independent.
• Apply Your Wound Care Skills: Assist with wound care needs, including wound vac management, when indicated.

• Active Nebraska Registered Nurse (RN) license
• Strong clinical assessment, communication, and organizational skills
• Confidence working independently and collaborating with an interdisciplinary team

• Home health experience required for the part-time weekend position
