Job description:
Job Overview
We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, develop, and implement personalized treatment plans to help patients regain mobility, reduce pain, and improve overall quality of life. Your expertise will support a diverse patient population across outpatient clinics, inpatient settings, and community-based care environments.
This position offers an exciting opportunity to make a meaningful difference by promoting healing and independence through evidence-based physiotherapy practices.
Responsibilities
- Conduct comprehensive patient evaluations to determine physical therapy needs, including assessments of physiology, anatomy, and functional mobility.
- Develop and implement tailored treatment plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities suited to each patient's condition.
- Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate care plans and ensure optimal patient outcomes.
- Manage electronic health records (EHR) or electronic medical records (EMR) systems accurately to document patient progress, treatment interventions, and discharge planning details in compliance with clinical documentation standards.
- Provide patient education on injury prevention, home exercise programs, and community resources to support ongoing rehabilitation outside clinical settings.
- Assist in discharge planning by evaluating patient progress and recommending appropriate follow-up care or referrals for specialized services such as pediatrics, neurology, orthopedics, or geriatrics.
- Support patients across various environments including outpatient clinics, inpatient hospital units, home health visits, and outpatient rehabilitation centers.
Qualifications
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience in practicing physical therapy within acute care, inpatient hospital settings, outpatient clinics, or home health environments.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for effective treatment planning and record keeping.
- Demonstrated ability to develop comprehensive care plans tailored to diverse populations such as geriatric patients, pediatric clients, or neurological cases.
- Experience working with electronic health records (EHR) systems or electronic medical records (EMR), including documentation of treatment plans and progress notes.
- Familiarity with outpatient rehabilitation techniques including orthopedics and trigger point therapy; experience with inpatient or hospital-based care is highly desirable.
- Excellent client communication skills to educate patients on their treatment process and foster a positive therapeutic relationship.
- Ability to work collaboratively with multidisciplinary teams to optimize patient recovery outcomes.
Join us in delivering exceptional physiotherapy services that empower patients on their journey toward improved mobility and well-being!
