Responsibilities
- Assess patients’ physical conditions through thorough evaluations, including anatomy and physiology knowledge, to inform treatment strategies.
- Develop and implement individualized care plans focused on patient rehabilitation, mobility improvement, and pain management.
- Deliver evidence-based physiotherapy interventions such as therapeutic exercises, trigger point therapy, and modalities suited for various conditions including orthopedics, neurology, pediatrics, and geriatrics.
- Manage clinical documentation standards by accurately recording treatment progress, updating care plans, and ensuring compliance with healthcare regulations using electronic health records (EHR) systems.
- Coordinate discharge planning and collaborate with healthcare professionals such as physicians, nurses, and social workers to optimize patient outcomes.
- Provide education to patients and their families on home exercise programs, safety precautions, and community resources for ongoing support.
- Conduct regular follow-up visits to monitor progress, modify treatment plans as needed, and ensure continuity of care in the home or outpatient settings.
Requirements
- Valid state licensure or certification as a licensed Physical Therapist with practicing in a physical therapy role.
- Proven experience in outpatient clinics, inpatient hospital settings, or home health environments with a focus on patient care and rehabilitation.
- Strong knowledge of medical terminology, physiology, anatomy, and clinical documentation standards.
- Experience working with diverse populations including geriatrics, pediatrics, orthopedics, neurology or those requiring outpatient rehabilitation.
- Familiarity with EMR (Electronic Medical Records) systems and outpatient clinic experience is highly preferred.
- Ability to perform discharge planning effectively while collaborating with healthcare teams to develop comprehensive treatment plans.
- Excellent client communication skills to educate patients about their care plans and promote engagement in their recovery process.
- Demonstrated ability to work independently in home & community care settings while adhering to safety protocols and clinical guidelines.

Pay: From $120,000.00 per year
