About the Role
We are seeking an experienced Occupational Therapist (OT) Home Health for a 13-week contract opportunity in Alameda County, California. The Occupational Therapist will provide patient-centered home health services while helping patients improve functional independence, safety, mobility, and performance of daily activities within their home environment.
This position requires strong time management, timely clinical documentation, supervision of Occupational Therapy Assistants, and effective communication with clinical and administrative teams. The productivity expectation is 30 points per week.
Responsibilities
- Evaluate patients in the home setting and develop individualized occupational therapy plans of care based on functional needs and clinical goals.
- Prioritize start-of-care visits and complete required clinical documentation within established timeframes, including next-day start-of-care reporting.
- Provide skilled occupational therapy interventions designed to improve activities of daily living, safety, functional independence, and quality of life.
- Supervise Occupational Therapy Assistants and maintain timely communication with supervisors, intake teams, and office staff.
- Maintain required productivity standards, complete documentation within 48 hours of visits, and travel throughout the assigned service territory as needed.
Requirements
- Active California Occupational Therapist (OT) License Required.
- Minimum 1 Year of Home Health Occupational Therapy Experience Required.
- Experience Managing Home Health Start-of-Care Visits and Supervising Occupational Therapy Assistants Required.
- Ability to Meet a 30-Point Weekly Productivity Requirement and Maintain Timely Clinical Documentation Required.
- Kinnser Documentation System Experience Preferred; Reliable Communication and Ability to Travel Throughout the Assigned Territory Required.
The Occupational Therapist will provide timely, compassionate, and patient-centered home health rehabilitation services while supporting safe functional independence in the home.
