About the Role

We are seeking an experienced Occupational Therapist (OT) Home Health for a 13-week contract in Missoula, Montana. The Occupational Therapist will evaluate patients in their homes, develop individualized plans of care, provide skilled rehabilitation interventions, and educate patients and families to improve function, independence, and safety.

Responsibilities

• Evaluate patients in the home setting and develop individualized occupational therapy plans of care based on functional needs and physician orders.
• Provide skilled occupational therapy treatment, reassess patient progress, and modify frequency, duration, and interventions as clinically appropriate.
• Complete required OASIS assessments and point-of-care documentation accurately and within established regulatory timelines.
• Coordinate patient care with physicians and interdisciplinary healthcare professionals while participating in care conferences, admissions, reassessments, and discharge planning.
• Supervise Occupational Therapy Assistant services as required and provide patient and caregiver education to support safety, independence, and continuity of care.

Requirements

• Minimum 2 Years of Occupational Therapist Experience Required; Home Health Experience Preferred.
• Current, Active Montana Occupational Therapist License Required.
• Bachelor s Degree or Higher From an Accredited Occupational Therapy Program Required.
• Current BLS/BCLS Certification Required.
• Valid Driver s License and Appropriate Auto Insurance Required; Strong OASIS, Home Health Documentation, Care Coordination, and Independent Travel Skills Required.
