The Californian-Pasadena is an 82-bed skilled nursing facility located directly across the street from Huntington Hospital. We are currently seeking a Full-Time Licensed Occupational Therapist (OTR/L) to join our rehabilitation team.

The Californian is a great setting for an Occupational Therapist who wants to grow clinically, collaborate with an experienced interdisciplinary team, and work with a diverse skilled nursing and post-acute population. Our rehabilitation team works closely with nursing, physicians, and other disciplines to help residents maximize their independence, safety, and quality of life.

What You'll Do

  • Perform comprehensive occupational therapy evaluations and develop individualized plans of care
  • Assess ADLs, functional mobility, cognition, upper-extremity function, balance, coordination, safety, and other areas impacting independence
  • Provide skilled interventions including ADL retraining, therapeutic exercise and activities, cognitive interventions, functional transfer training, and adaptive techniques
  • Recommend and train residents in the use of adaptive equipment and environmental modifications when appropriate
  • Establish meaningful, functional goals based on each resident's needs and prior level of function
  • Utilize standardized assessments and objective measures to monitor progress and demonstrate outcomes
  • Monitor resident response to treatment and modify plans of care as clinically appropriate
  • Provide resident, family, and caregiver education to improve safety and carryover
  • Collaborate closely with PT, SLP, nursing, physicians, and the interdisciplinary team
  • Participate in care planning and discharge planning to promote safe transitions
  • Complete accurate and timely clinical documentation

Qualifications

  • Current California Occupational Therapist license – required
  • CPR/BLS certification
  • Skilled nursing, post-acute, or long-term care experience preferred, but newer clinicians who are motivated to learn and grow are encouraged to apply
  • Strong clinical reasoning, communication, and documentation skills
  • Ability to work collaboratively within an interdisciplinary team
  • Commitment to compassionate, resident-centered care
