We’re seeking a licensed Physical Therapist to provide in-home care to pediatric and adult patients. This role is ideal for a clinician who enjoys working independently while helping patients improve mobility, strength, function, and independence.

Responsibilities

• Perform in-home physical therapy evaluations.
• Develop individualized treatment plans and functional goals.
• Provide therapy to improve mobility, strength, and daily functioning.
• Monitor patient progress and adjust treatment plans as needed.
• Coordinate care with PTAs, physicians, and other members of the clinical team.
• Complete accurate and timely clinical documentation.

Qualifications

• Active Texas Physical Therapist license.
• Minimum of one year of PT experience.
• Current BLS/CPR certification.
• Own device and reliable internet connection required to access your schedule and complete charting.
• Valid driver’s license and reliable transportation.
• Strong clinical, organizational, and communication skills.
• Ability to work independently.
