This opportunity is ideal for nurses with strong clinical judgment, acute care experience, and the ability to work independently while providing personalized, one-on-one patient care.

• Pay Rate: $45–$52 per hour
• Location: Cleveland, Ohio, and surrounding areas
• Shifts: Morning and overnight shifts available
• Schedule: Flexible options based on patient and program needs
• Service Area: Candidates within approximately 30 miles of Cleveland preferred

Key Responsibilities

• Assess patients and monitor vital signs, symptoms, and overall condition.
• Administer medications and provide skilled nursing care, including IV therapy and wound care, as applicable.
• Implement physician orders and individualized care plans.
• Recognize changes in patient condition and respond appropriately to emergencies.
• Coordinate care with physicians and healthcare professionals.
• Educate patients and families on medications, treatments, and disease management.
• Maintain accurate documentation and follow patient safety, infection control, and HIPAA standards.

QualificationsRequirements

EducationExperience

• Active, unrestricted Ohio Registered Nurse (RN) license
• Minimum of 1–2 years of RN experience
• Home health, ER, ICU, medical-surgical, or acute care experience strongly preferred
• IV therapy, wound care, and complex patient care experience preferred

Required Credentials

• Current CPR certification
• Negative TB test or chest X-ray, in accordance with agency requirements
• Required immunization records or approved exemptions, as applicable
• Ability to pass required background checks and drug screening

Additional Requirements

• Preferably located within 30 miles of Cleveland
• Valid driver's license, reliable transportation, and current auto insurance
• Willingness to travel to patients' homes throughout the service area
• Strong clinical judgment, communication, and critical-thinking skills
• Ability to work independently and use electronic medical records and mobile charting systems
