JOB SUMMARY:

The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their homes in accordance with physician orders and the established plan of care. The OT evaluates patients' functional abilities and develops individualized treatment plans to help patients achieve their highest level of independence and function within their capabilities.

• Travel to multiple locations to provide occupational therapy services to patients in their homes in a time-efficient manner.
• Complete initial evaluations and assessments to determine patients' functional abilities, needs, and goals.
• Develop, implement, and modify individualized plans of care in accordance with physician orders.
• Provide skilled occupational therapy interventions to improve or minimize physical limitations and promote independence with daily activities.
• Instruct patients and family members in home programs, therapeutic activities, exercises, and techniques to improve function and safety.
• Complete reassessments and modify treatment plans as necessary based on patient progress and needs.
• Communicate with physicians and other members of the care team regarding patient progress, changes in condition, and recommended changes to treatment.
• Prepare accurate and timely clinical documentation, including evaluations, progress notes, reassessments, and discharge summaries.
• Collaborate with nursing, therapy, and other home health team members in patient care planning.
• Advise and consult with patients, families, and Agency personnel regarding the patient's plan of care.
• Participate in Agency in-service programs and quality improvement activities as assigned.

POSITION QUALIFICATIONS:

• New Grads welcome!
• One (1) to two (2) years of recent acute care experience in an institutional setting.
• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in the State of Illinois.
• National certification/registration as applicable.
• Two (2) years of occupational therapy experience preferred.
• Excellent observation, verbal and written communication, and problem-solving skills.
• Ability to work independently while effectively collaborating with physicians and other members of the interdisciplinary care team.
• Computer skills adequate for completion of electronic clinical documentation.
• Current Illinois Driver's License and proof of current auto insurance.

LOCATIONS

We are looking for an OT to support our therapy team in Elgin and surrounding communities.

Pay: $80.00 - $90.00 per hour
