Absolute Home Wellness is seeking Registered Nurses to join our growing home health team! We have part-time and PRN opportunities available for nurses interested in providing one-on-one patient care throughout DeKalb, Sycamore, and neighboring communities.

What You'll Do

As a Home Health RN, you'll visit patients in their homes to assess their health, provide skilled nursing services, and help them manage their medical conditions.

Your daily responsibilities may include:

• Performing initial nursing assessments and admissions
• Completing scheduled skilled nursing visits
• Monitoring patient health and identifying changes in condition
• Providing nursing treatments and interventions based on physician orders
• Teaching patients and caregivers about medications, disease management, and home care
• Communicating with physicians and clinical leadership regarding patient needs
• Participating in care planning and coordinating services with other healthcare professionals
• Completing electronic visit documentation accurately and within required timeframes
• Helping patients maintain their health, safety, and independence at home

Who We're Looking For

Qualifications:

• Active Illinois Registered Nurse (RN) license
• Graduation from an accredited nursing program
• Current CPR certification
• Valid driver's license and active auto insurance
• Comfortable traveling locally to patient homes
• Strong communication and organizational skills
• Ability to work independently and manage patient care responsibilities
• Basic computer proficiency for electronic charting
• Willingness to learn and demonstrate clinical nursing skills

Previous home health experience is not required. We offer training and support to help nurses become comfortable providing care in the home setting.
