Location: Oakland & Surrounding Areas, CA

Contract Duration: 13 Weeks

Schedule: Day Shift

Guaranteed Hours: 36 Hours/Week (subject to productivity requirements)

Weekly Productivity: Minimum 27 points

Service Areas

• Area 2: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont
• Area 8: Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington

Additional travel outside the assigned areas and weekend coverage may be required.

Job Description

We are seeking an experienced Physical Therapist to provide home health therapy services. The therapist will evaluate patients, develop individualized treatment plans, and deliver care across the assigned service areas.

Requirements

• Active California Physical Therapist license.
• Current BLS certification.
• Valid driver's license.
• Personal vehicle with current auto insurance and registration.
• Must be willing to see COVID-positive and COVID-PUI patients.
• Must be flexible with travel outside the assigned territory and weekend coverage when needed.
• Required health clearances: TB, MMR, Varicella, Hepatitis B, Tdap, annual Flu vaccination, drug screen, and physical.

Responsibilities

• Conduct patient evaluations and reassessments.
• Develop and implement individualized treatment plans.
• Provide home health physical therapy services.
• Complete OASIS assessments and required documentation.
• Monitor patient progress and update care plans.
• Coordinate care with physicians and other healthcare professionals.
• Complete patient visits while meeting weekly productivity expectations.

Important Assignment Details

• Payment is based on completed visits and the facility's point-to-hour conversion.
• The 36-hour guarantee requires completion of at least 27 points per week and is subject to facility conditions.
• Travel time, patient visits, documentation, and calls are included in the all-inclusive rate.
• All requested time off (RTO) must be disclosed at submission.
• Former employees of the facility are not eligible for travel assignments.
