Company Overview
Xincon Home Health Care Services is hiring a per-diem Licensed Occupational Therapist for the CAPABLE (Community Aging in Place, Advancing Better Living for Elders) program, a Johns Hopkins University evidence-based initiative that helps older adults live independently in their homes.
Job Summary
This is a per-visit position, not hourly. You will provide up to 6 in-home visits per participant over approximately 4–5 months, working with older adults to assess their home environment, set personalized goals, and implement strategies that improve daily functioning and independence. Compensation is $100 per completed home visit.
Visit volume depends on caseload availability.
Duties
- Conduct in-home assessments with older adults enrolled in the CAPABLE program
- Use Motivational Interviewing techniques to help participants identify and prioritize their own goals for greater independence
- Assess functional performance, safety, and environmental barriers in each participant's home
- Develop up to 3 personalized Action Plans per participant
- Create prioritized Work Orders for home modifications and coordinate directly with the Handy Worker
- Collaborate closely with the CAPABLE Registered Nurse and Program Coordinator throughout the participant's enrollment
- Reinforce problem-solving strategies and provide education and resources across sessions
- Maintain and manage adaptive equipment; assist in resolving participant concerns related to home modifications
- Document visits and maintain accurate records according to program and organizational guidelines
Required Qualifications
- Education: Bachelor's or Master's degree in Occupational Therapy from an accredited program
- Licensure: Valid New York State Occupational Therapy license (Required)
- Experience: At least 1 year of experience as a licensed Occupational Therapist (Required)
- Willingness to complete required CAPABLE certification training through Johns Hopkins University (training provided)
- Comfortable conducting independent in-home visits and managing your own schedule
- Familiarity with medical documentation standards and electronic health records (EHR) management systems.
- Excellent communication skills for client interaction and interdisciplinary team collaboration.
- Ability to adapt therapy approaches based on individual patient needs and progress updates.
Nice to have
- Experience in home care with older adults
- Bilingual: English + Chinese or Spanish
- Familiarity with client-centered, collaborative care models
