Home Health RN Charlottesville VA

Welcome Homecare is seeking Full-Time, and Part-Time Home Health Registered Nurses to assist our patients in Charlottesville VA area.

· Assess and chart observations of the patient’s condition at each visit.

· Complete evaluation tasks, including reviewing medication and vital signs.

· Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.

Requirements:

• Current RN license
• At least one (1) year of recent home health experience
• Able to work in the community, drive and meet patients in their home
• Must be authorized to work in the US
