VIGEO Home Health is hiring Virginia-licensed Occupational Therapists for PRN home health visits across Northern Virginia. This role is for OTs who want flexible supplemental work, a defined service area, and one-on-one patient care without facility productivity pressure or double-booking.
This is a PRN W-2 employee position. Clinicians share the days, times, and local areas they can cover. Assignments are offered based on patient needs, physician orders, geography, availability, and clinical fit.

Our Northern Virginia service areas include Alexandria, Annandale, Arlington, Ashburn, Aldie, Burke, Centreville, Chantilly, Fairfax, Fairfax Station, Falls Church, Gainesville, Great Falls, Herndon, Leesburg, McLean, Reston, Sterling, Springfield, Vienna, and Woodbridge. VIGEO confirms ZIP codes and preferred coverage areas before assigning visits. Patient assignments are grouped geographically whenever possible.
Any coverage outside the agreed territory is reviewed before assignment.
Compensation
Structure
Pay is $80-$150 per completed visit, based on visit type and final rate review. VIGEO uses a W-2 pay-per-visit model, not a guaranteed hourly schedule. Typical visits are approximately 45-60 minutes.
Actual time can vary based on patient needs, visit type, documentation, travel, and clinical circumstances. Mileage reimbursement is available according to VIGEO's current reimbursement policy. Final visit categories and rates are reviewed before hire.
Patient volume can vary by territory, referrals, census, clinical fit, and clinician availability.
Onboarding And Training
All VIGEO clinicians complete a structured two-month onboarding pathway as they ramp into active visits. This includes paid required orientation, EMR and documentation training, competency validation, compliance training, and role-specific clinical support. Timing and visit assignment ramp may vary based on credentialing, availability, role requirements, territory need, and clinical readiness.
What You Will Do
- Complete OT evaluations, reassessments, recertification visits, and discharge planning
- Develop and update plans of care for ADLs, upper-extremity function, cognition, safety, home access, adaptive equipment, and independence
- Deliver skilled OT interventions for ADLs, fine motor function, upper extremity recovery, cognition, energy conservation, adaptive techniques, and home safety
- Provide patient and caregiver education, home programs, adaptive-equipment recommendations, and home-safety training
- Coordinate changes in condition and treatment recommendations with physicians and the care team
- Supervise COTAs and Home Health Aides when assigned and permitted by the plan of care
- Complete timely documentation in the agency's electronic medical record
Requirements
- Current Virginia Occupational Therapist license in good standing
- Bachelor's, Master's, or Doctorate in Occupational Therapy from an ACOTE-accredited program
- Current CPR certification
- Valid driver's license, reliable transportation, and current automobile insurance
- Ability to travel to patient homes in the agreed Northern Virginia service area
- Ability to complete the structured two-month onboarding pathway, including EMR, competency, documentation, and compliance training
- Comfortable accepting at least 8 visits per month once active
Strongly Preferred
- Home health or Medicare-certified home health experience
- 1-2+ years of clinical experience
- NBCOT certification
- Experience with geriatric, orthopedic, neurological, post-hospital, cognitive, ADL, fall-risk, and home-safety needs
- Experience using WellSky/Kinnser or a comparable home health EMR
