Uplift HomeCare is seeking a licensed Physical Therapist (PT) to serve patients in West Tampa and surrounding communities!

We provide compassionate, personalized in-home care to seniors and veterans, helping patients improve mobility, regain function, and maintain independence in the comfort of their homes.

Responsibilities

• Evaluate patients and develop individualized treatment plans and therapy goals.
• Provide skilled physical therapy interventions to improve strength, balance, mobility, and functional independence.
• Perform required assessments and reevaluations and monitor patient progress.
• Educate patients and caregivers on therapeutic exercises, safe mobility, fall prevention, and home safety.
• Communicate patient progress and changes in condition to physicians and the clinical team.
• Coordinate care with nurses, occupational therapists, and other healthcare professionals.
• Complete accurate, timely clinical documentation and required assessments.
• Promote safe, effective, patient-centered care in the home environment.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active Florida Physical Therapist license.
• Current CPR certification.
• AHCA Level II background screening.
• Home health experience preferred.
• Strong clinical judgment, assessment, communication, and documentation skills.
• Reliable transportation and ability to travel to patient homes.
• Ability to work independently while collaborating with the clinical team.
