Location: Springfield, OH 45504

Shift: Days (8:00 AM – 5:00 PM)

Job Summary

We are looking for a qualified Physical Therapist (PT) to provide quality patient care through home health services in Springfield, Ohio. The ideal candidate will have at least 1 year of experience and be comfortable providing individualized therapy services to patients in their homes to improve mobility, strength, balance, and overall functional independence.

Responsibilities

• Evaluate patients' physical conditions, mobility, strength, and functional abilities.
• Develop individualized treatment plans based on patients' needs and goals.
• Provide physical therapy treatments in patients' homes.
• Help patients improve mobility, balance, coordination, and muscle strength.
• Assist patients with exercises and techniques to improve daily functional activities.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and caregivers on home exercise programs and safety practices.
• Recommend appropriate mobility aids and assistive devices when necessary.
• Maintain accurate and timely patient documentation.
• Coordinate care with physicians and other healthcare professionals.

Required Certifications

BLS Certification – Required

Must-Have Experience & Skills

• Minimum 1 year of Physical Therapy experience – Required
• Strong patient assessment and treatment-planning skills.
• Knowledge of therapeutic exercises and rehabilitation techniques.
• Ability to work independently and manage patient care effectively.
• Excellent communication and patient-education skills.
• Compassionate approach to patient care.

• Shift: 8:00 AM – 5:00 PM
• Shift Type: Day Shift
