This will be in our skilled nursing facility where you will be at the forefront of delivering compassionate, evidence-based physiotherapy care that makes a meaningful difference every day.
Duties
- Conduct comprehensive evaluations of patients’ physical conditions, including mobility, strength, and functional limitations.
- Develop and execute tailored treatment plans utilizing techniques such as trigger point therapy, orthopedics, neurology interventions, and geriatrics care.
- Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate patient care and discharge planning.
- Manage clinical documentation standards by accurately recording treatment progress, patient responses, and care plans within electronic health records (EHR) or electronic medical records (EMR) systems.
- Assist patients with rehabilitation exercises and therapies to improve mobility, strength, and overall function—especially focusing on geriatric populations.
- Provide education to patients and their families about physiotherapy techniques, injury prevention, and home & community care strategies.
- Ensure compliance with hospital policies, outpatient clinic protocols, and outpatient rehabilitation center standards while maintaining high-quality patient care.
Skills
- Proven experience practicing in a physical therapy role within skilled nursing facilities.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for effective treatment planning.
- Skilled in discharge planning processes and working with geriatrics.
- Ability to develop comprehensive care plans that incorporate physiotherapy techniques such as trigger point therapy and other manual therapies.
- Excellent client communication skills to educate patients on treatment goals and foster positive therapeutic relationships.
- Experience working with geriatric patients in skilled nursing facilities.

Pay: $50.00 - $60.00 per hour
Expected hours: 2.0 – 20.0 per week
