Physical Therapist

We are a growing Home Health Agency looking for good Physical Therapist for PRN/Contract work in the Houston and Greater Houston Metropolitan Area.

We are looking for committed Physical Therapists for adult and geriatric patient services to be seen in their home settings.

Responsibilities

• Identify and meet patients’ goals and needs
• Offer cost-effective individualized treatment plan
• Create fitness- and wellness-oriented programs tailored to patients’ specific needs
• Provide quality, personalized and evidence-based care and proven interventions
• Motivate patients during treatment in order to help them function optimally
• Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination
• Consult and collaborate with other healthcare team professionals
• Evaluate effects , monitor and communicate progress and modify treatment plans appropriately.
• Educate patient, family and caregivers as needed and provide Home Exercises Program
• Document accurate patient care services

Skills

• Graduate degree or greater in Physical Therapy
• Valid Texas license to practice Physical Therapy
• Proven Home Health work experience as a Physical Therapist
• Drive to continually learn and grow
• Reliable Transportation, Valid diver's license and current auto insurance
• Professional behavior
• Excellent interpersonal communication skill
• Bilingual a plus

Job Type: Contract

Pay: Up to $350.00 per day
