Position

Full-Time Home Health Registered Nurse (RN)

Responsibilities

• Perform comprehensive Start of Care (SOC), Recertification, Resumption of Care (ROC), Discharge, and Skilled Nursing visits.
• Complete accurate and timely OASIS assessments.
• Develop, implement, and update individualized Plans of Care in collaboration with physicians and the interdisciplinary team.
• Provide skilled nursing care and patient education for chronic disease management, medication management, wound care, IV therapy, catheter care, and other clinical services.
• Monitor patient progress and communicate changes in condition to physicians and appropriate team members.
• Ensure compliance with Medicare Conditions of Participation, state regulations, and agency policies.
• Maintain complete, accurate, and timely documentation in Homecare Homebase (HCHB).
• Participate in case conferences, quality improvement initiatives, and interdisciplinary team meetings.
• Educate patients and caregivers regarding disease processes, medications, safety, infection prevention, and emergency preparedness.

QualificationsRequired

• Current Registered Nurse (RN) license.
• Minimum 2 years of recent Home Health experience.
• Previous Homecare Homebase (HCHB) experience is required.
• Strong knowledge of OASIS documentation.
• Excellent assessment and critical thinking skills.
• Current BLS certification.
• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to work independently while managing a daily patient schedule.

Preferred

• Experience with Medicare home health regulations.
• Wound care certification or advanced wound care experience.
• IV therapy/PICC line experience.
• Knowledge of PDGM and home health reimbursement.
• Experience with quality measures and value-based care.
