### **Your Impactful Role as a Physical Therapist**

In your role, you will:

  • **Provide One\-on\-One Care:** Evaluate and treat patients in their homes, developing individualized treatment plans focused on mobility, function, independence, and quality of life.
  • **Manage Your Schedule:** Enjoy the autonomy of managing your patient visits while traveling throughout your assigned service area.
  • **Help Patients Reach Their Goals:** Use your clinical expertise to help patients improve strength, balance, mobility, and functional independence.
  • **Collaborate with the Care Team:** Work closely with nurses, occupational therapists, aides, social workers, physicians, patients, and families to coordinate comprehensive care.
  • **Educate Patients and Caregivers:** Provide education and instruction on therapeutic exercises, mobility, safety, and strategies that support continued progress at home.
  • **Document and Communicate:** Maintain timely, accurate clinical documentation and communicate patient progress and changes in condition to the appropriate members of the care team.

### **What You'll Need to Excel**

  • **Licensure:** A current Minnesota Physical Therapist license.
  • **Education:** Graduate degree from an accredited Physical Therapy program.
  • **Patient\-Centered Mindset:** A genuine passion for helping patients achieve their goals and maintain independence.
  • **Clinical Expertise:** Strong assessment, treatment planning, and clinical decision\-making skills.
  • **Independence:** The ability to work confidently and effectively in a home health environment.
  • **Teamwork:** A willingness to collaborate with patients, families, caregivers, and members of the interdisciplinary team.
  • **Driver's License:** A valid driver's license and reliable transportation.
