This is a PRN, evaluation-focused role: you complete evaluations, 30-day reassessments, recertifications, and discharges, and our licensed PTAs complete follow-up treatment visits under your plan of care.
We provide home and community-based services to adults across Arizona. Our multidisciplinary team collaborates to deliver individualized, evidence-based care that meets each patient/client where they are.
Responsibilities
- Complete PT evaluations, 30-day reassessments, recertifications, and discharges (start-of-care/OASIS visits are completed by the agency)
- Develop individualized plans of care targeting mobility, gait, balance, strength, transfers, and fall-risk reduction
- Supervise and collaborate with the PTAs who complete follow-up treatment visits, and adjust the plan of care as needed
- Collaborate with patient/client, families, and the care team
- Complete accurate, timely visit documentation
Requirements
- Active Arizona Physical Therapist license in good standing.
- Current CPR/BLS certification.
- Arizona Level One Fingerprint Clearance Card (or ability to obtain prior to start).
- Valid Arizona driver's license.
- Current professional liability (malpractice) insurance (required for 1099 contractors).
- Welcome to apply if no experience in home-based care.
- Ability to work independently while maintaining effective communication with the interdisciplinary team.
- Strong clinical reasoning, organizational, and time management skills.
- Excellent written and verbal communication skills.
- Ability to complete documentation accurately and within required timelines.
Preferred Qualifications
- Experience working with adults in home-based or community settings.
- Experience in home health, outpatient, or community-based rehabilitation.
Benefits
- Flexible PRN schedule.
- Tiered per-visit pay that reflects visit complexity.
- Collaborative and supportive interdisciplinary team environment.
