Job description

About this role

If you’re a Physical Therapist (PT) who wants to empower patients to achieve meaningful outcomes through compassionate and patient-centered therapy—this is it.

In this role, you’ll provide physical therapy services to the adult/geriatric population, working closely with patients to improve independence, mobility, safety, and overall quality of life. You’ll have the opportunity to follow patient progress over time and deliver truly patient-centered care in a collaborative clinical environment.

Here’s your chance to focus on what matters most—quality outcomes, consistency of care, and clinical autonomy while making a meaningful difference every day. Why This Role Stands Out:* Build meaningful relationships while helping individuals improve independence, mobility and overall quality of life

  • Enjoy flexible scheduling options and a supportive work environment designed to promote work-life balance
  • Provide personalized, patient-centered physical therapy focused on functional outcomes
  • Deliver high-quality care in a setting that values clinical excellence over high patient volume
  • Thrive in a collaborative, therapist-driven environment that encourages professional growth, mentorship and clinical autonomy
  • Evaluate patients and provide personalized physical therapy treatments that help patients improve mobility, strength, balance, and functional independence
  • Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals
  • Deliver hands-on therapy interventions designed to support recovery, increased endurance, and enhance overall quality of life in older adults
  • Help patients restore functional mobility and safely perform daily activities with greater confidence and independence
  • Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care
  • Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals

Compensation & Benefits:* Competitive Compensation and Benefits

Qualifications:* Licensed Physical Therapist (PT) or license eligible in the state of practice

  • Graduate of an accredited Physical Therapist Program
  • Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
  • Passion for patient-centered care and improving quality of life for older adults
