The successful candidate will be responsible for providing high-quality therapy services in a home health setting, ensuring that patients achieve their rehabilitation goals.

Key Responsibilities

• Conduct evaluations and develop customized treatment plans for patients.
• Provide therapy sessions in a home setting, typically averaging four visits per day.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Document patient progress and adjust treatment plans as necessary.
• Educate patients and their families on strategies and exercises to enhance their recovery.

Qualifications

• Minimum of 2 years of experience in a healthcare setting is required.
• Current Occupational Therapy license in California.
• BLS Certification is mandatory.
• National Provider Identifier (NPI) number is required.
• A valid driver's license and proof of automobile insurance are needed for this position.
• COTAs with home health experience may be considered if no OT applicants are available.
• Work Schedule This position is Monday through Friday, with daily hours from 8:30 AM to 5:00 PM.
• The therapist will be the first to float as required.
• Travel Requirements The coverage area includes northeastern Sacramento into Folsom and Woodland, with services provided in the Yolo County area.
• Mileage will be reimbursed at the GSA rate with prior approval.
