Three Rivers Hospice is seeking a compassionate and dedicated Registered Nurse (RN) Case Manager to join our team serving St. Joseph, Missouri, and the surrounding communities.

What You'll Do

• Provide personalized nursing care to hospice patients in their homes and nursing facilities.
• Develop and coordinate individualized plans of care.
• Assess patient needs and manage pain and symptoms to promote comfort.
• Educate and support patients and families throughout their hospice journey.
• Collaborate with physicians and the interdisciplinary hospice team.
• Maintain accurate and timely clinical documentation.
• Build meaningful relationships with patients and their families.

What We're Looking For

• Active Missouri RN license
• Minimum of 1–2 years of nursing experience; hospice or home health experience preferred.
• Strong assessment, organizational, and communication skills.
• Ability to work independently while being part of a supportive team.
• Compassionate, dependable, and patient-focused approach.
• Valid driver's license, reliable transportation, and current auto insurance.
