We are seeking a **licensed Physical Therapist** to provide **home health services** on a **PRN basis**. The ideal candidate will be passionate about **patient care**, committed to **rehabilitation**, and eager to work in a **flexible, supportive environment**.
**Coverage Areas: Garland, TX**
**Duties:**
- Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
- Assesses for muscle strength, mobility, gait, ROM\-potential for rehab.
- Directing the PT treatment, instructing patients and families/caregivers in the use and care of therapeutic appliances.
- Document patient progress and communicate with other healthcare professionals
- Adhere to all state and federal regulations and guidelines
**Requirements:**
- Licensed Physical Therapist in the state of TEXAS
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Ability to commute is a must
