What you’ll do

· Provide OT evaluations and treatment in clients’ homes and senior living communities

· Deliver specialized lymphedema assessment and treatment

· Create individualized plans of care that improve ADLs, independence, safety, mobility, and quality of life

· Educate patients and caregivers on compression, skin care, positioning, and home programs

· Collaborate with physicians, nurses, families, case managers, and facility staff

· Recognize when clients could benefit from additional PT and/or Speech Therapy services

· Complete timely, accurate documentation

What you bring

· Current Wisconsin OT license in good standing

· Strong assessment, communication, and documentation skills

· Comfort working independently in a community-based setting

· Reliable transportation and willingness to travel within your territory

· Comfort with electronic documentation and technology
