Physical Therapist

Job Overview

As a Physical Therapist, you’ll help patients improve movement, manage functional challenges, and work toward meaningful rehabilitation goals. You’ll assess each patient’s needs, develop individualized treatment plans, and coordinate care across appropriate settings, including hospitals, clinics, outpatient rehabilitation centers, and home and community care.

Responsibilities

• Evaluate patients using knowledge of anatomy, physiology, and medical terminology; identify functional needs and rehabilitation goals.
• Develop and update care plans, deliver physiotherapy interventions, and support patient rehabilitation, including trigger point therapy when appropriate.
• Coordinate discharge planning and collaborate with healthcare professionals to support continuity of patient care.
• Maintain accurate physical therapy documentation in electronic health records (EHR) or other electronic medical record (EMR) systems, following clinical documentation standards.
• Communicate treatment progress and next steps clearly with patients, families, and care teams.

Qualifications

• Experience practicing in a physical therapy role and providing patient care.
• Relevant experience in acute care, inpatient or hospital settings, outpatient clinics or rehabilitation centers, home health, or home and community care.
• Ability to work with geriatric patients; experience with pediatrics, neurology, orthopedics, or geriatrics is also valued.
• Knowledge of treatment plan development, care plans, clinical documentation standards, and physical therapy documentation.
• Strong client communication skills and familiarity with EHR or EMR management; experience across clinic or outpatient settings is preferred.

Pay: $45.00 - $55.00 per hour

Expected hours: 5.0 – 40.0 per week
