Home Health Occupational Therapist (OT)

We are seeking a **Home Health Occupational Therapist (OT)** who values flexibility, efficient workflows, and a supportive team that understands what home health is actually like. **Duties**

• Evaluate patients’ functional abilities and develop individualized plans of care focused on ADLs and IADLs
• Provide skilled occupational therapy interventions to improve safety, mobility, and independence in the home
• Address areas such as dressing, grooming, toileting, transfers, and functional mobility
• Assess home environments and recommend adaptive equipment or safety modifications as appropriate
• Educate patients and caregivers on strategies to promote independence and reduce fall risk
• Communicate patient progress and clinical changes with the interdisciplinary care team and physician as needed
• Maintain accurate, timely electronic documentation in compliance with home health and regulatory standards

**Qualifications**

• Active and unrestricted California Occupational Therapist license
• Graduate of an accredited Occupational Therapy program
• Home health experience preferred, but not required
• Strong understanding of ADLs, functional mobility, and patient safety
• Excellent communication and time\-management skills
• Ability to work independently in a home\-based setting
• Reliable transportation and valid driver’s license

**Position Details**

• **Pay:** $85 – $135 per visit
• **Schedule:** Your own hours
• **Coverage Area:** Your preference coverage in Ventura County
• **Work Location:** In person (home health)
