Position Overview

We are seeking an experienced

Occupational Therapist (OT)

to join our Home Health team. In this role, you will provide in-home occupational therapy services, evaluating and treating patients in the comfort of their homes to help improve functional independence, safety, and quality of life.

Key Responsibilities

• Conduct comprehensive occupational therapy evaluations in patients' homes.
• Develop and implement individualized treatment plans based on patient needs and goals.
• Provide skilled therapeutic interventions to improve daily living skills, mobility, strength, coordination, and overall function.
• Educate patients, families, and caregivers on treatment plans, home safety, adaptive equipment, and strategies to maximize independence.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
• Complete accurate and timely documentation in Homecare Homebase (HCHB).
• Monitor patient progress and adjust treatment plans as needed.
• Ensure compliance with all clinical, regulatory, and agency standards.

Qualifications

Required:

• Active Occupational Therapist (OT) License
• Home Health experience
• Experience using Homecare Homebase (HCHB)
• Strong clinical assessment and patient care skills
• Ability to work independently in a field-based environment
