Timely assessment and coordinated care help patients and families make informed decisions and support safer transitions between care settings.

- Maintain clear medical documentation and electronic health records (EHRs), following privacy requirements under the Health Insurance Portability and Accountability Act (HIPAA).
- Coordinate with clinicians and support staff to provide patient-centered care in settings such as hospitals, clinics, long-term care, home care, and senior care.
Requirements
- Active registered nurse license required; current Basic Life Support (BLS) certification required where applicable.
- Demonstrated nursing skills in patient assessment, medication administration, IV insertion, aseptic technique, medical documentation, and patient care.
- Familiarity with anatomy, physiology, medical terminology, vital signs, and electronic medical record (EMR) or EHR systems; platforms may include Epic, Cerner, Athenahealth, or eClinicalWorks.
- Relevant experience may include acute, inpatient, emergency, intensive care, medical-surgical, pediatrics, geriatrics, behavioral health, dialysis, hospice, or post-acute care.
- Additional setting-specific experience or skills are valued, including ophthalmology, trauma, cardiac catheterization, labor and delivery, operating room care, memory care, developmental disabilities, triage, phlebotomy, suturing, and ICD coding (International Classification of Diseases, Ninth and Tenth Revisions).
