Public Health Nurse/RN (Full Time)

Essential Duties and Responsibilities:

· Provide case management for high-risk pregnant women and infants, including assessing needs, developing individualized care plans, coordinating services and referrals, and following up to support access to care and community resources.

· Conduct home visits to assess maternal and infant needs, provide individualized health education and support, identify barriers to care, and connect families with appropriate services.

· Perform physical and nursing assessments of pregnant women and infants within the nursing scope of practice and agency protocols; document findings, identify concerns, and coordinate appropriate referrals and follow-up.

· Provide immunization services in accordance with agency policies and clinical protocols, including reviewing immunization histories, assessing eligibility, educating clients and caregivers, administering vaccines, and maintaining accurate records.

· Complete age-appropriate developmental screenings using program-approved tools, discuss results with caregivers, provide education on developmental milestones, and facilitate referrals and follow-up when concerns are identified.

· Provide other assigned public health nursing and clinical services within the nursing scope of practice, consistent with agency policies and program requirements, including client education, care coordination, timely documentation, and collaboration with the care team.

Education and/or Experience: -Licensure in the state of Illinois as a Registered Professional Nurse required; BSN preferred. -Experience in and knowledge of public health nursing or population focused nursing preferred.

Hours

Full time, 37.5 hours/week with the possibility of occasional evenings and/or weekends.

Salary: $25.77-26.80 per hour
