Position Summary

We are seeking an experienced, compassionate, and dependable Registered Nurse (RN) to join our growing home health and hospice team in the San Antonio area. The ideal candidate will have at least two years of Home Health and /or Hospice nursing experience, strong clinical and documentation skills, and the ability to work independently while providing high-quality patient-centered care.

• Administer medications, treatments, and wound care as prescribed.
• Monitor patient progress and communicate updates to physicians and care teams.
• Educate patients and families on health management and disease prevention.
• Maintain accurate documentation in compliance with regulatory standards.
• Coordinate care with physicians, caregivers and the interdisciplinary team
• Perform comprehensive patient assessments and on going evaluations
• Develop, implement, and update individualized plans of care
• Provide skilled nursing care in patients home
• Deliver hospice and palliative care focused on comfort, dignity, and quality of life
• Provide patient and family education regarding medications, treatments, symptom management, and safety
• Complete accurate and timely documentation
• Participant in admissions, recertifications, resumptions of care, and discharges as assigned
• Communicate changes in patient condition promptly to the appropriate provider and clinical team
• Maintain compliance with agency policies an applicable state and federal requirements
• Travel throughout San Antonio and surrounding service areas as needed

QUALIFICATIONS

• Current and unrestricted Registered Nurse (RN) license in Texas or applicable compact license
• Minimum 2 years of Home Health and/or Hospice experience required
• Strong clinical assessment, critical-thinking, organizational, and communication skills
• knowledge of Medicare Home Health and/or Hospice requirements preferred
• OASIS experience strongly preferred for Home Health candidates
• Hospice case-management experience is a plus
• Current CPR/BLS certification
• Valid Driver's license, reliable transportation, and current automobile insurance
• Willing and able to travel throughout the assigned service area
• Bilingual English/Spanish is a plus
