Physical Therapist

Job Summary

We are seeking a Physical Therapist to support patients in improving movement, function, and independence. You will assess patient needs, develop individualized treatment plans, and coordinate care across appropriate settings, including outpatient clinics, hospitals, and home and community care. This role calls for sound clinical judgment, clear communication, and a commitment to safe, patient-centered rehabilitation.

Duties

• Evaluate patients’ mobility and functional needs, applying knowledge of anatomy and physiology to guide care.
• Develop and update care plans and treatment plans; provide physiotherapy and rehabilitation, including orthopedic interventions and trigger point therapy when appropriate.
• Your impact: Thoughtful treatment planning and discharge planning help patients and their care teams prepare for safe, coordinated transitions.
• Collaborate with healthcare professionals, communicate with patients and caregivers, and document care in electronic health records (EHR) or other electronic medical record (EMR) systems.
• Provide patient care across relevant populations and settings, which may include geriatrics, pediatrics, neurology, acute care, inpatient, outpatient, and home health services.

Experience

• Experience practicing in a physical therapy role, with patient rehabilitation and physical therapy documentation skills.
• Familiarity with clinical documentation standards, medical terminology, and EHR/EMR management.
• Relevant experience may include acute care, hospital, inpatient, clinic, outpatient clinic, outpatient rehabilitation center, home health, or home and community care.
• Clear client communication and the ability to coordinate effectively with healthcare professionals are essential.
