Physical Therapist (PT) — PRN / Part-Time, Paid Per Visit — Fox Trail Memory Care, Chester, NJ

Swift Rehabilitation Inc is a mobile outpatient rehabilitation practice providing physical, occupational, and speech therapy on-site at patients' homes, assisted living facilities, and adult daycares across Maryland and New Jersey. We are not a home health agency.

We are seeking a Physical Therapist for a single-site, local position at Fox Trail Memory Care, 116 Route 206, Chester, NJ 07930, an assisted living / memory care facility. This is NOT an adult daycare position. You will treat residents on-site at this one facility only — no travel between multiple sites for this role.

The caseload starts with 3 residents cleared to start immediately and is expected to grow. This is an ideal local opportunity for a therapist who lives near Chester, NJ and wants a stable, single-facility caseload.

Pay: $55-$65 per visit, paid per visit (PRN / part-time). Please note: pay is per visit, not hourly.

What you will do:

• Provide on-site physical therapy evaluations and treatments for memory-care residents at Fox Trail Memory Care in Chester, NJ
• Develop and carry out individualized plans of care focused on mobility, strength, balance, safety, and functional independence
• Document evaluations, treatments, and progress notes in WebPT EMR
• Coordinate care with facility nursing staff and residents' families
• Communicate changes in condition and progress clearly and promptly

Requirements:

• Active New Jersey physical therapist license, or PT Compact privilege to practice in New Jersey
• Current CPR/BLS certification
• Professional liability coverage
• Reliable transportation
• Experience with geriatric / memory-care patients preferred

Schedule: PRN / part-time, based on resident need and caseload growth, starting immediately.
