About the Role

Supportive Care Specialists is hiring a full-time or part-time Palliative Care Nurse Practitioner in Arcadia, CA. You’ll provide compassionate, coordinated care for people living with serious illness, supporting comfort, informed choices, and quality of life. You’ll work closely with patients, families, and healthcare professionals across care settings.

Timely assessment and coordinated symptom care help patients and families make informed decisions and feel more comfortable.

What You’ll Do

• Assess symptoms, health history, vital signs, and care goals, and complete physical exams and diagnostic evaluations within your scope of practice
• Develop and update individualized care plans, including pain and symptom management, medication administration, and prescribing as authorized
• Coordinate with patients, families, physicians, nursing teams, hospice services, and community providers, and support case management and discharge planning
• Document assessments, treatment plans, and follow-up in the EMR/EHR, following HIPAA privacy requirements and applicable clinical standards
• Follow applicable DEA regulations

What You Need

• Active California Nurse Practitioner license and authorization to practice
• Advanced practice experience in hospice and palliative medicine
• Strong patient assessment, clinical documentation, communication, and collaboration skills
• Knowledge of anatomy, physiology, medical terminology, and medication safety

Nice to Have

• Experience in geriatrics, senior care, home care, nursing homes, assisted living, hospitals, inpatient or outpatient care, or dementia and memory care
• EMR/EHR experience (Epic, Cerner, Athenahealth, or eClinicalWorks)
• Experience with telehealth, triage, acute care, pain management, or case management
