Sun Valley Home Healthcare is seeking a motivated, compassionate, and experienced RN Nursing Supervisor for a unique hybrid position combining hands-on patient care, clinical supervision, staff development, and in-office operational leadership.

What You'll Do

Field-Based Clinical Responsibilities

• Perform skilled nursing visits, including SOC, ROC, recertifications, and routine patient visits.
• Conduct supervisory visits and clinical competency evaluations.
• Provide hands-on training, mentoring, and orientation for clinical staff.
• Support timely patient admissions, including weekend coverage as needed.
• Ensure quality patient care and accurate clinical documentation.

In-Office Leadership & Operations

• Collaborate with the Director of Nursing and Administrator on daily clinical operations.
• Assist with clinical scheduling, staffing coordination, and patient assignments.
• Review OASIS assessments, physician orders, and clinical documentation.
• Support quality assurance, regulatory compliance, and performance improvement initiatives.
• Help resolve patient care concerns and support communication with physicians and referral partners.
• Promote clinical staff accountability, productivity, and professional development.

What We're Looking For

Required Qualifications

• Active Arizona RN license.
• Minimum 4 years of professional nursing experience.
• Minimum 2 years in Home Healthcare
• Strong clinical assessment, leadership, and communication skills.
• Excellent organizational and problem-solving abilities.
• Comfortable working independently in the field and collaboratively in the office.
• Reliable transportation and willingness to travel throughout the Phoenix metropolitan area.
• Medicare-certified home healthcare experience. (2yrs)
• Knowledge of OASIS documentation and CMS regulations. (Required)
• Prior clinical supervisory, training, or leadership experience.
• Familiarity with home health EMR systems.
