**Occupational Therapist (OT) – Mobile Outpatient \| Niagara County, NY \| Part\-Time**

Stern at Home Therapy is hiring a **Part\-Time Occupational Therapist (OT)** to provide individualized, one\-on\-one care to patients throughout **Niagara County, NY**.

This is an excellent opportunity for an Occupational Therapist looking for a **consistent part\-time caseload, flexible scheduling, and strong clinical autonomy** while treating patients one\-on\-one in their homes.

With a minimum commitment of **10 patients**, therapists can build a geographically concentrated caseload within Niagara County while receiving dedicated administrative support for referrals, scheduling, authorizations, and patient coordination. **What You'll Do:**

As an Occupational Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes, helping patients improve or maintain independence with daily activities and functional tasks. **Responsibilities Include:*** Perform occupational therapy evaluations and develop individualized plans of care.

• Provide one\-on\-one occupational therapy treatments in patients' homes.
• Help patients improve independence with activities of daily living.
• Address upper\-extremity strength, range of motion, coordination, cognition, safety, and overall functional performance.
• Provide therapeutic activities and exercises based on individual patient goals.
• Recommend adaptive equipment, assistive devices, and home modifications when appropriate.
• Educate patients and caregivers on adaptive strategies, exercises, and safety.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Complete accurate and timely clinical documentation.
• Communicate with physicians, caregivers, families, and interdisciplinary care team members.

**Compensation:****Part\-Time – Minimum 10 Patients** **Part\-Time W\-2:*** **$55 per follow\-up visit. **

• **$65 per 4\-unit evaluation.**
• Fixed per\-visit compensation with no tiered volume model.

**Part\-Time 1099:*** **$62 per follow\-up visit. **

• **$75 per evaluation.**
• Fixed per\-visit compensation with no tiered volume model.

**Part\-Time W\-2 Benefits:*** Health insurance and/or medical stipend based on eligibility.

**Qualifications:*** Active Occupational Therapist license in New York required.

• Graduate of an accredited Occupational Therapy program.
• Reliable transportation required.
• Strong clinical, communication, documentation, and time\-management skills.
• Ability to work independently in a home\-based environment.
• Home health or geriatric experience preferred but not required.
