##### **Occupational Therapist (OT) – Home Health \| North Buffalo \& Cheektowaga, NY \| Part\-Time**

##### **Stern at Home Therapy is hiring a** **Part\-Time Occupational Therapist (OT)** **to provide individualized, one\-on\-one care to patients throughout** **North Buffalo, Cheektowaga, and surrounding communities****. **

##### **This is an excellent opportunity for an Occupational Therapist looking for a** **consistent part\-time caseload with flexible scheduling and strong clinical autonomy****. **

##### **With a minimum commitment of** **10 patients****, therapists can build a geographically concentrated caseload across the North Buffalo and Cheektowaga area while receiving dedicated administrative support for referrals, scheduling, authorizations, and patient coordination. **

##### **What You'll Do:**

##### **As an Occupational Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes, helping patients improve or maintain independence with daily activities and functional tasks. **

##### **Responsibilities include:**

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide one\-on\-one occupational therapy treatments in patients' homes.
• Help patients improve independence with activities of daily living.
• Address upper\-extremity strength, range of motion, coordination, cognition, safety, and overall functional performance.
• Provide therapeutic activities and exercises based on individual patient goals.
• Recommend adaptive equipment, assistive devices, and home modifications when appropriate.
• Educate patients and caregivers on adaptive strategies, exercises, and safety.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Complete accurate and timely clinical documentation.
• Communicate with physicians, caregivers, families, and interdisciplinary care team members.

##### **Compensation:**

• **Part\-Time – Minimum 10 Patients.**
• **Part\-Time W\-2\.**
• **$55 per follow\-up visit.**
• **$65 per 4\-unit evaluation.**
• Fixed per\-visit compensation with no tiered volume model.

##### **Part\-Time 1099:**

• **$62 per follow\-up visit.**
• **$75 per evaluation.**
• Fixed per\-visit compensation with no tiered volume model.

##### **Part\-Time W\-2 Benefits**

• Health insurance and/or medical stipend based on eligibility.
• 401(k) with annual employer match.

##### **Qualifications:**

• Active Occupational Therapist license in New York required.
• Graduate of an accredited Occupational Therapy program.
• Reliable transportation required.
• Strong clinical, communication, documentation, and time\-management skills.
• Ability to work independently in a home\-based environment.
• Home health or geriatric experience preferred but not required.
