As a Federally Qualified Health Center (FQHC), SWHS is committed to providing quality, accessible, and affordable healthcare to the communities we serve. We are looking for a PMHNP who shares our commitment to patient-centered care and improving access to mental health services for individuals and families.

Position Overview

The PMHNP provides psychiatric evaluations, diagnosis, treatment, and medication management for patients of all ages. • Diagnose and treat mental health and substance use disorders using evidence-based practices.
  • Develop individualized treatment plans and provide ongoing psychiatric medication management.
  • Monitor treatment effectiveness, medication adherence, and patient progress.
  • Assess suicide risk and other behavioral health emergencies, providing appropriate interventions and safety planning.
  • Collaborate with primary care providers, Licensed Clinical Social Workers, and other healthcare professionals to coordinate patient care.
  • Educate patients and families about mental health conditions, medications, and treatment options.
  • Coordinate referrals and follow-up care to support continuity of services.
  • Maintain accurate and timely documentation in the electronic health record.
  • Participate in quality improvement, patient safety, and interdisciplinary care initiatives.

Education & Qualifications

  • Master's degree or Doctor of Nursing Practice (DNP) degree from an accredited nursing program with preparation as a Psychiatric Mental Health Nurse Practitioner.
  • Current, unrestricted RN license recognized by the North Carolina Board of Nursing.
  • Current North Carolina Nurse Practitioner approval to practice, or eligibility to obtain approval before beginning clinical duties.
  • National board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC).
  • DEA registration and applicable prescribing authority, or eligibility to obtain required registrations.
  • Current Basic Life Support (BLS) certification.
  • Ability to meet organizational credentialing and privileging requirements.

Preferred Experience

  • Experience providing psychiatric evaluations and medication management.
  • Experience in community health, integrated behavioral health, primary care, or an FQHC setting.
  • Experience working with patients across the lifespan and serving diverse or underserved populations.

What We're Looking For

  • We are seeking a provider who demonstrates sound clinical judgment, compassion, strong communication skills, and a collaborative approach to patient care.
  • The ideal candidate is committed to building meaningful relationships with patients, addressing barriers to treatment, and working as part of a team to improve health outcomes.
  • Why Join SWHS?
