Responsibilities:

• Design and implement individualized physical therapy treatment plans to promote optimal physical function and independence for patients
• Conduct thorough assessments and screenings to identify physical therapy needs and develop goals for patients
• Develop and implement effective treatment plans that address the physical, emotional, and social needs of patients
• Collaborate with patients, families, and interdisciplinary teams to ensure seamless care coordination
• Provide education and training to patients, families, and caregivers on physical therapy techniques and strategies
• Maintain accurate and thorough records of patient assessments, treatments, and progress
• Participate in quality improvement initiatives to enhance patient care and outcomes
• Stay current with latest evidence-based practices and industry developments to ensure best practices
