Nurse Practitioner

Job Summary

SoCal Nursing Wound Care, PC is seeking an experienced and motivated Nurse Practitioner to provide advanced mobile wound care services to patients in home health, hospice, assisted living, and residential settings. This role focuses on delivering high-quality, evidence-based wound care directly to patients where they live while collaborating with home health agencies, hospice teams, caregivers, and physicians.

The ideal candidate is confident working independently in the field, has strong clinical judgment, and is passionate about improving wound healing outcomes and patient quality of life.

*We are looking for someone to start out part-time (2 days per week, one of those days being a Friday, other day can be flexible) 2 hours a week and eventually move to a full time role.

*Starting pay range is based on experience and hourly rate will be up for negotiation based on RVU's after 90 days.

Responsibilities

• Perform comprehensive wound assessments and ongoing evaluations
• Develop and implement individualized wound treatment plans
• Perform procedures including sharp/mechanical debridement, Ultramist therapy, dressing applications, and advanced wound therapies
• Apply and manage skin substitutes and other advanced wound care modalities as appropriate
• Document visits accurately and thoroughly in the EMR (NextGen), communicate with team via Roam, update schedule on Google Sheets, be available via text and email during normal business hours.
• Coordinate care with physicians, home health agencies, hospice teams, and caregivers
• Educate patients and families on wound prevention and treatment plans
• Monitor treatment progress and adjust plans of care accordingly
• Maintain compliance with Medicare and regulatory documentation standards

Requirements

• Active California Nurse Practitioner license
• Board Certification as a Nurse Practitioner
• Strong assessment and procedural skills
• Ability to work independently and efficiently in a mobile setting
• Excellent communication and documentation skills
• Valid driver’s license and reliable transportation
• Prefer previous wound care experience

Pay: $65.00 - $80.00 per hour
