Silver Oak Nursing & Rehabilitation Center is seeking a compassionate, dedicated, and skilled Per Diem Physical Therapist to join our team and provide high-quality rehabilitation services to our residents.

Position Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans
- Provide therapeutic exercises, functional mobility training, gait training, balance activities, and other appropriate interventions
- Monitor and document residents’ progress and response to treatment
- Modify treatment plans as needed based on clinical findings and resident progress
- Collaborate with the interdisciplinary team to support quality resident care
- Educate residents, families, and caregivers on appropriate exercises, mobility techniques, and safety strategies
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements
- Promote resident safety, independence, dignity, and quality of life
Qualifications
- Current Physical Therapist (PT) license in the state of Iowa
- Graduate of an accredited Physical Therapy program
- Strong clinical, communication, and organizational skills
- Experience in skilled nursing, long-term care, rehabilitation, or geriatrics preferred
- Compassionate and patient-centered approach to care
- Ability to work independently while collaborating effectively with an interdisciplinary team
