Physical Therapist

We are currently seeking a Physical Therapists (PT) to join our team. In this role, you will support your patients by providing direct care and coordinating with the interdisciplinary team to ensure that both patients and their families have an optimal care experience. Responsibilities

• Provide skilled physical therapy care as outlined in the patient care plan
• Submit clinical notes, progress notes, and other clinical record forms, outlining the services rendered, as indicated
• Participate in the patient's discharge planning process
• Adhere to federal, state, and accreditation requirements, including Medicare and Medicaid regulations

Pay: $45.00 - $47.00 per hour
