**Overview**
Join our dynamic healthcare team as a Full\-Time, Part\-Time, or PRN (as needed) Physical Therapist dedicated to enhancing patient mobility, independence, and quality of life in a short\-term and long\-term skilled nursing facility. In this vital role, you will deliver expert physiotherapy services in a skilled nursing rehabilitation setting.
Your expertise will support a wide range of patient needs in orthopedic recoveries, neurological rehabilitation, and geriatric skilled nursing ensuring comprehensive, personalized treatment plans. **Responsibilities**
- Develop and implement tailored treatment plans for patients across various medical conditions, ensuring alignment with clinical standards and patient goals
- Conduct thorough assessments of patients’ physical function, mobility, and pain levels to inform effective therapy strategies
- Collaborate closely with healthcare professionals to coordinate care plans and optimize patient outcomes
- Manage detailed clinical documentation in electronic health records (EHR) systems while adhering to medical terminology standards and documentation protocols
- Assist with discharge planning by evaluating patient progress and recommending appropriate follow\-up care or community resources
- Provide hands\-on therapy interventions such as trigger point therapy, therapeutic exercises, manual techniques, and modalities suited to individual needs
- Work with diverse populations including geriatrics, orthopedics, neurology, and skilled nursing patients, and those in home health or outpatient rehabilitation settings
**Skills**
- Proven experience practicing in a physical therapy role within outpatient hospital settings, outpatient clinics, or home health environments
- Strong knowledge of physiology, anatomy, orthopedics, neurology, geriatrics, and skilled nursing to inform effective treatment strategies
- Familiarity with outpatient rehabilitation center operations and outpatient clinic experience preferred
- Skilled in developing comprehensive care plans that incorporate discharge planning and patient education
- Proficient in medical terminology and compliant with clinical documentation standards for accurate recordkeeping
- Ability to collaborate effectively with healthcare teams to develop treatment plans that meet individual patient needs
- Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for seamless documentation management
- Knowledge of physiotherapy techniques including trigger point therapy and modalities for patient rehabilitation
- Excellent client communication skills to educate patients about their treatment process and motivate engagement in their recovery journey
- Background working with geriatric patients in home \& community care settings or outpatient environments is highly valued

Pay: $40\. 00 \- $60\. 00 per hour
