Physical Therapist

We are seeking a compassionate and motivated PRN Physical Therapist (PT) to join our team and provide high\-quality, patient\-centered care. One year of physical therapy experience is preferred; however, new graduates are welcome to apply. An active Physical Therapist license in the state of Arizona is required.

The ideal candidate will have strong communication skills, a commitment to patient care.

**Pay – Paid Daily**

• SOC / PT Evaluation: $110
• Routine Visit: $80
• Recertification: $95
• Discharge: $85
• Resumption of Care: $100

**What You’ll Do**

• Perform PT evaluations and home health visits
• Complete recertifications, discharges, and resumptions of care
• Coordinate with the RN and physician as needed
• Complete documentation promptly in our EMR

**Requirements**

• Active Arizona Physical Therapist license
• Reliable transportation
• Comfortable driving throughout the Phoenix Valley
• Home health experience preferred, but not required
