Duties
- Conduct comprehensive assessments of patients’ physical function, mobility, and pain levels within home environments or outpatient clinics.
- Develop and implement individualized care plans aligned with clinical standards, patient goals, and healthcare provider directives.
- Deliver evidence-based physiotherapy interventions including therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities suitable for diverse conditions like orthopedics, neurology, pediatrics, and geriatrics.
- Manage discharge planning processes by coordinating with healthcare professionals to ensure seamless transitions and continuity of care.
- Maintain accurate and detailed clinical documentation in electronic health records (EHR) systems, adhering to medical terminology and documentation standards.
- Collaborate closely with physicians, nurses, occupational therapists, and other healthcare providers to optimize treatment strategies and patient outcomes.
- Educate patients and their families on injury prevention, home exercise programs, medication management, and community resources to support ongoing recovery.
Experience
- Proven practice experience in physical therapy roles within outpatient clinics, inpatient hospitals, or home health settings.
- Demonstrated expertise working with geriatric populations as well as pediatric patients across various rehabilitation needs.
- Strong background in discharge planning, treatment plan development, and clinical documentation standards.
- Familiarity with outpatient rehabilitation center operations and outpatient clinic experience preferred.
- Knowledge of physiology, anatomy, medical terminology, and specialized therapies such as trigger point therapy is essential.
- Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for accurate documentation and reporting.
- Prior involvement in acute care or inpatient hospital environments enhances understanding of complex medical conditions requiring interdisciplinary coordination.

Pay: $50. 00 - $60. 00 per hour
