Occupational Therapist (In-Home/Field-Based)

Provide compassionate occupational therapy to home health and hospice patients in their homes. Assess patients’ functional status and needs, develop and implement individualized treatment plans, and support safe mobility, independence, and quality of life.

Key Responsibilities

• Conduct functional assessments and establish individualized OT treatment goals.
• Deliver therapeutic interventions to improve safety, daily living skills, and independence.
• Collaborate with an interdisciplinary care team to coordinate effective care.
• Educate patients, families, and caregivers on strategies and home safety.
• Document evaluations, treatments, and outcomes in an EMR system.

Qualifications & Skills

• Occupational therapy licensure/credentials required to practice.
• Strong clinical judgment and ability to individualize treatment plans.
• Excellent communication and caregiver education skills.
• Comfort working independently in a home-based care setting.
• Proficiency with EMR documentation and attention to clinical detail.
