Job Description

Heal At Home is seeking an experienced Nurse Practitioner (NP) to join our growing healthcare team.

This position focuses on Chronic Wound Care, Primary Care, and clinical oversight of Chronic Care Management (CCM) services.

The role is primarily remote, involving patient evaluations, telehealth consultations, supervision of clinical staff, and review of patient care plans. Occasional home visits will be required for wound assessments and treatments.

Key Responsibilities

Chronic Wound Care

• Evaluate and treat patients with chronic and non-healing wounds.
• Develop and manage individualized wound treatment plans.
• Perform occasional home visits for wound assessments and treatments.
• Monitor wound healing and adjust treatment plans as needed.
• Coordinate care with physicians, home health agencies, and specialists.

Chronic Care Management (CCM) & Primary Care

• Provide clinical oversight of CCM services performed by designated clinical staff.
• Review, approve, and sign off on patient care plans and clinical documentation as appropriate.
• Supervise clinical staff and provide guidance on patient conditions, medications, and treatment plans.
• Conduct remote patient evaluations and telehealth consultations when clinically necessary.
• Address escalated clinical concerns and make medical decisions regarding patient care.
• Coordinate with primary care physicians and specialists to ensure continuity of care.
• Ensure appropriate clinical documentation and compliance with applicable care management requirements.

Qualifications

• Active Ohio Nurse Practitioner license.
• Chronic wound care experience required.
• Experience in primary care or chronic disease management preferred.
• Experience supervising clinical staff or overseeing CCM programs is a plus.
• Comfortable conducting remote patient evaluations.
• Strong communication, organizational, and clinical decision-making skills.
• Ability to work independently and occasionally travel for patient home visits.
